Application Procedures

Correction in Birth Certificate

A.   INSTITUTIONAL BIRTH	A correction letter is required from Medical Record Officer of concerned hospital/institution where birth/death event occurred.
B.   NON-INSTITUTIONAL BIRTH	i) Affidavit stamp paper on Rs.10/- duly attested by SDM/Notary public having subject correction of birth events ii) Supporting document like Passport, Ration Card, Voter Identity Card, 10th class certificate etc.

Application Procedures

Entry of Child Name in Birth Record
1	Within One Year: Hand written application to Registrar birth having subject name inclusion of child by the concerned child parent and to be submitted at all birth centers.	Rs.3/-
2	After One Year: i) Hand written application to Registrar birth having subject n Nagar penchant Ashrafpur kichucha ame inclusion of child by the concerned child parent and to be ii) Affidavit stamp paper on Rs.10/- duly attested by notary public having subject name inclusion of child	Rs.5/- As late fee + Affidavit

For Issuance Of Birth Certificate

A.      INSTITUTIONAL BIRTH

1. Application procedure	Birth application form duly filled by head of the family/Relative etc which are available at Nagar penchant Asrafpur kichucha). The forms shall be available on the Nagar Panchyat Ashrafpur kichucha Website also.
2.  Fee Structure	For first copy of birth certificate Rs.25/-

B. HOME BIRTH

1. Application procedure	Birth application form duly filled by head of the family/Relative etc which are available at Nagar Panchyat Ashrafpur kichucha Concerned Areas Birth and Death. The forms shall be available on the Website Nagar Panchyat ashrafpur kichucha also.
2.  Fee Structure	For first copy of birth certificate Rs25/-

Documents Required

Following Documents are Required for Registration of Home Birth with in One Year of its Occurrence:

1. Duly filled birth reporting form by head of the family or relatives.
2. Certificate of Doctor etc. who assist in delivery of child.

Beyond one year of occurrence of birth event the registration will be done after submission of S.D.M order from local area along with Rs. 5/- as late fee

1	Within 21 days of the occurrence of Birth	Rupees 3/-
2	After 21 days to 30 days of the occurrence of Birth	Rs.5/- As late fee
3	After 30 days to within 1 year of the occurrence of Birth	Rs.5/- As late fee
4	Beyond One year of the occurrence of Birth	SDM Order from local area + Rs.5/- As late fee
